Overview
Establish and maintain therapeutic relationships with patients and families and provide support following established care plans. Provide personal care including bathing, showering, grooming, dressing, oral hygiene, toileting, skin care, and assisting with meals/feeding as required. Perform nursing care functions as applicable, including ensuring awareness of Kardex, risk triggers, signature behaviors, and unit milieu; communicate reports and records in the patient chart. Respond to call bells/alarms by alerting appropriate team members and assist as necessary. Respond to emergent situations by calling appropriate code and assisting the care team according to unit/program guidelines. Observe and monitor patient progress and behavioral changes; report significant observations (e.g., discomfort/pain, condition/complaints/accidents) to the Registered Nurse or designate as applicable
