Overview
Provide expertise to support staff/residents/families with integration of residents with cognitive/responsive behaviours into the Long Term Care (LTC) environment. Assist the Home’s collaborative Behaviour Support Resource Team. Share best practices with nursing staff and support resident care plans. Liaise with external resources and outreach teams for interventions, support and follow-through with recommendations. Participate in an LTC community of practice to share and improve knowledge in care of responsive and complex behaviours. Complete documentation and reports as required. Provide education in-services related to skills and knowledge improvement. Assess meaning, etiology and inherent risk of behaviours using an objective, systematic and holistic process. Critically assess and identify caregiver strategies/interventions that are abilities focused, person-centred and age appropriate. Support staff to develop and implement care interventions for resident responsive behaviours. Act as a resource to other staff and initiate a systematic approach to identifying and resolving problems regarding residents' care and well-being. Provide guidance and work direction to staff regarding implementing clinical care programs, policies and procedures. Collaborate on interventions for resident behaviours. Participate in Responsive Behaviour Program planning, implementation and evaluation to ensure evidence-based practice and continuous quality improvement. Develop and/or maintain professional working relationships with internal and external partners. Educate staff and coordinate education such as PIECES model training; monitor and ensure appropriate education to prevent/reduce responsive behaviours. Maintain required certifications as defined by legislation and operational requirements. May respond to emergency situations outside normal business hours as related to the Departmental Emergency Plan
