Assess nutritional needs of selected patients and develop comprehensive nutrition care plans. Recommend nutrient requirements, feeding choices, and delivery methods including enteral feeds. Monitor and adjust nutrition care plans in response to biochemical and other nutrition-related data. Consult with physicians and other healthcare professionals regarding patient response to nutrition care plans. Communicate nutrition care plans to other providers, family, and/or caregivers in hospital or community. Provide nutrition counseling and training for patients and caregivers, including specialized formulas and food preparation. Develop written education materials and resources in collaboration with relevant teams. Complete and maintain related records (e.g., documentation on charts, workload statistics, formula requisitions, menus, and patient care databases). Support program operation evaluation and goal/objective setting from a clinical nutrition perspective by reviewing caseload, trends, statistics, and resource utilization
