The Registered Practical Nurse provides holistic, compassionate nursing care to residents while maintaining documentation standards and care plans. They also assume a leadership role by mentoring staff and ensuring compliance with safety and nursing practice standards.

  • Week 2: Mon (D) Sat (D) Sun (D) This is a part-time, permanent 0.4 FTE or 30 hours bi-weekly Registered Practical Nurse (RPN) position starting as soon as possible. Overview Under the supervision of the Retirement Home Care Teampervisor, the Registered Practical Nurse (RPN) is responsible for the provision and management of holistic, inclusive, compassionate quality nursing care to residents of the Carefor Retirement Home. RPN will assume a leadership role by teaching, delegating, mentoring, and providing guidance to different levels of workers within the care team. Care is provided within the framework of quality & risk management, Carefor policies and procedures, standards and scope of practice set by the College of Nurses of Ontario (CNO), and other identified Best Practices and Directives.

Responsibilities

Provides complete, relevant assessment of the resident and describes and/or documents according to documentation standards of practice. In collaboration with the Manager or designate, prepares and updates the resident health assessment and plan of care. Effectively and safely administers medication according to Carefor procedures and CNO standards of practice Collaborates and advocates with other members of the health care team to ensure residents receive quality care in a timely manner. Identifies problems using a specified framework, critical thinking, and in consultation with the Manager or designate and resident, selects and implements individualized strategies or interventions in order to focus care on resolving or managing the problem Describes, evaluates and documents resident’s responses to care accurately, clearly and comprehensively using objective and subjective data. Describes rationale for referrals and changes in Plan of Care. Provides organized, outcome directed care on an ongoing basis Identifies and utilizes tools and resources for work and manages these resources through regular monitoring of equipment etc. and reporting unusable equipment to Supervisor or designate. Delegates care appropriately Meets with residents and family to orient them to the residence. Seeks input from residents to assure that service meets expectations Plans, implements, evaluates and adjusts care in accordance with resident focused expectations Functions in accordance with the standards of nursing practice from the College of Nurses of Ontario Functions in accordance with the Carefor policies and procedures and the strategic direction, Occupational Health and Safety legislation, RHRA legislation and regulations, Infection Prevention and Control practices, and any other pertinent practice and directive. Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger him/herself or another worker Reports any violations of the Health and Safety Act or the regulations or the existence of any hazard Ensures all workplace incidents are reported in a timely manner in accordance with Event Management policies and procedures.

Qualifications

RPN Diploma from approved college Valid registration with the College of Nurses of Ontario Certificate in gerontology would be an asset. Current CPR and First Aid 3-5 years’ experience Experience in a long-term care or retirement facility an asset Experience working with people who have dementia, Alzheimer’s Disease, and mental health illnesses

Requirements

Candidates must hold an RPN diploma and valid registration with the College of Nurses of Ontario. The role requires 3-5 years of experience, preferably in a long-term care or retirement setting, along with current CPR and First Aid certification.
