Key function
- To provide assertive Community Outreach and actively participate on a multidisciplinary team to provide direct client services for people who are living with serious, complex mental illness, and who require long-term support.
- REPORTING:Under the general direction of the Clinical Manager, Psychosocial Rehabilitation.
Duties
1. Provide evidence-based treatment for individuals in the Psychosocial Rehabilitation programs.2. Complete intake assessments. 3. Develop and implement intervention plans based on best practice within a team approach to minimize or eliminate identified impacts and problems.4. Document assessments, treatment plans, progress notes, and outcomes in a timely manner.5. Assist in and act as a resource for client and family education. 6. Consult and liaise with appropriate resources to ensure quality care and maintain patient safety.7. Provide guidance, training and orientation for new staff and students.8. Work collaboratively with community providers to provide wrap around care for complex clients. 9. Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.10. Apply the Standards of Professional Practice as outlined by the College of the applicable regulatory body.11. Maintain standards of patient care as outlined for the progam. 12. Maintain and adhere to HSN policies, procedures and standards of care.13. Support the Medical Professional in systematically assessing the health status of the individual by ordering, interpreting, and evaluating diagnostic tests.14. Develop plan of care with the help of other healthcare professionals to meet patient’s needs and communicate actions to maintain continuity among the nursing team. 15. Systematically and continuously evaluate the extent to which the individual’s health needs are being met and modify plan of care as indicated by patient’s response and condition.16. Provide patient and family with counselling, advise on health maintenance, and arrange for the continuation of required care after patient discharge; refer to community resources as required.17. Attend regular in-service sessions, conferences and seminars to keep abreast of new developments, trends and techniques in the nursing field, attending sessions related to specialties as required. 18. Coordinate and utilize identified resources within the hospital including bed utilization and early discharge planning in the provision of safe quality patient care on the unit.19. Participate in long-term program goals, quality control activities and process improvement initiatives as required.20. Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes. 21. Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.22. Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.23. Educate and promote health, safety and wellness in the work place. 24. Represent the department or program on various committees and in meetings as required.25. Perform other duties as required. QUALIFICATIONSEDUCATION AND TRAINING:1. Current Certificate of Registration in good standing with the applicable regulatory college as a Registered Nurse (RN), Occupational Therapist (OT), Masters of Social Work (MSW), Psychological Associate, or Psychotherapist. 2. Certification in Cognitive Behavioral Therapy, including forty (40) hours of theory and supervision hours, is preferred.3. Additional training in Addictions and Counselling and Treatment is preferred.4. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required. EXPERIENCE:1. Minimum three (3) years of recent, relevant experience in providing psychiatric assessment, treatment, monitoring and evaluation of patients in a community setting.2. Minimum of one (1) year recent experience managing and developing community treatment orders (CTO). 3. Experience in community-based, outpatient mental health programs is required. KNOWLEDGE/SKILLS/ABILITIES:1. Demonstrated ability to practice in the areas of Psychotherapies with adults and families.2. Demonstrated Health Care knowledge of psychiatric conditions and the Diagnostic and Statistical Manual (DSM-IV). 3. Demonstrated skills in Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and individual andgroup interventions.4. Demonstrated knowledge and understanding of principles of recovery as it applies to mental health and addictions treatment.5. Demonstrated ability to manage rapidly changing mental health states and psychiatric emergencies. 6. Demonstrated skills in a range of individual and group psychotherapies, as well as experiential therapies for thetreatment of addictions & mental illness.7. Demonstrated excellent judgment and proven analytical skills.8. Demonstrate strong client and family service orientation. 9. Demonstrated knowledge of culturally safe practices.10. Demonstrated training, experience or utilization of quality improvement methodology for process improvement.11. Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement. 12. Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.13. Demonstrated superior interpersonal and communication skills, both written and verbal.14. Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required. 15. Demonstrated discretion and maturity when handling confidential information.16. Demonstrated commitment to the safety of co-workers and patients. PERSONAL SUITABILITY:1.
