About the role

The Social Worker provides direct patient care, including psycho-social assessments, crisis intervention, and family counseling. They also manage discharge planning and act as a liaison between patients and community support agencies.

As a member of the healthcare team, the Social Worker provides direct patient care through the provision of psycho-social screening and assessments; individual and family counseling and treatment; crisis intervention; bereavement counseling; liaison and referral to the community agencies; advocacy and discharge planning.

  • Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker
  • Master's Degree in Social Work from an accredited School of Social Work.
  • Knowledge of the Healthcare Consent Act, Substitute Decisions Act, Child and Services Act
  • Clinical SW experience preferred
  • Oncology and counselling experience preferred
  • Excellent knowledge of community support and placement services
  • Excellent crisis intervention skills
  • Demonstrated knowledge and experience in end of life issues
  • Awareness and counselling experience with quality of life issues related to treatment, side effect and chronic nature of disease
  • Excellent knowledge of community support and placement services
  • Proven ability to organize and prioritize patient caseload across several programs
  • Proven ability to work as a member of interdisciplinary healthcare teams
  • Proven ability to organize and prioritize patient caseload across several programs
  • Excellent communication skills
  • Hospital-based clinical experience in a variety of patient care areas preferred

Requirements

Candidates must hold a Master's Degree in Social Work and a current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers. Clinical experience in a hospital setting and knowledge of relevant healthcare legislation are required.
