Salary Range

The salary range for this position is $37.00 to $45.51 per hour, inclusive of a $5.00 per hour premium.

What you will do

As a Registered Nurse with the Integrated Care Program, you will help deliver a model of care that brings advanced, coordinated support directly from the hospital into the home. The Acute Care Registered Nurse delivers advanced assessments, monitoring, complex treatments, and rapid interventions for clients with serious illnesses and exacerbations of chronic conditions. These clients are expected to have complex, unstable, and unpredictable health conditions, with higher risks of adverse outcomes.

  • When performing work on behalf of the CBI Home Health Integrated Care Team, Registered Nurses are required to consistently demonstrate competency to perform Nursing skills to full scope, which may normally include:
  • PIV initiation and management
  • PICC line infusions and management
  • ABPI and compression
  • Symptom management following outlined pathways for diagnoses
  • VAC therapy and complex wound management
  • Thorough head to toe assessments and recognizing subtle changes to health status
  • Accurate, timely documentation in AlayaCare, Teams and admission / discharge assessments
  • Interpretation of lab results
  • Comfortable with infusion of complex medications
  • Triage and flag abnormal labs and vitals
  • Coordinate services; ie: specimen pick-up, in-home labs, and hospital appointments
  • Respond proactively to changes in patient health
  • Complete in-home visits - scheduled and ad hoc within shift

What you will need to be successful

Registered Nurse with the College of Nurses of Ontario, in good standing.

Ability to work the days and times specified in the shift rotation.

Flexibility to complete Acute care program related duties as assigned/ needed within shift

Attend regular and ad hoc interdisciplinary huddles and meetings.

Commitment to attend additional training as needed.

Experience providing acute, complex and/or hospital-level nursing care.

Strong clinical assessment, critical thinking, and decision-making skills with ability to recognize and respond to changes in patient condition.

Experience with IV therapy, vascular access devices, medication administration, complex wound and drain care and acute care monitoring.

Strong digital literacy with ability to quickly learn and navigate digital health technologies, including electronic health records, remote patient monitoring and point-of-care testing. Experience with Meditech or Epic preferred.

Current and valid BCLS Training required

Clear criminal record check with vulnerable sector screening

Proof of negative TB test within the last 12 months

Professional Liability Insurance (RNAO)

Reliable transportation and willingness to service all areas of Peterborough County, as operationally required.

A valid driver’s license, reliable vehicle and vehicle insurance required.

Strong verbal and written English communication skills, French or additional language skills considered an asset.
