The Social Worker will provide clinical support to geriatric clients with mental health and addiction issues within an interprofessional team. They are responsible for delivering culturally competent care and collaborating on complex care and recovery plans.
The Geriatric Mental Health Outpatient and Inpatient Services is currently seeking a part time, permanent Social Worker 2 to work in the Complex Care and Recovery Program. Reporting to the manager of Geriatric Mental Health Services, you will be expected to work collaboratively within an interprofessional team setting. Your target population of clients will be 65 years of age and older, with mental health and addiction issues, often with co-occurring medical issues.

Candidates must hold a Master of Social Work degree and be registered with the Ontario College of Social Workers and Social Service Workers. Proven experience in geriatric social work and knowledge of the aging process are considered significant assets.
