4 New or Replacement Position: Replacement Site: West Park Healthcare Centre Department: 4D Long Term Ventilation Reports to: Manager, Program Services Wage Range: $38.42 - $39.02 per hour (start rate in accordance with collective agreement) Shifts: Days/Evenings Hours: 20hr/week Status: Permanent Part-Time

Position Summary

As a member of a multi-disciplinary Team, the Registered Practical Nurse practices in accordance with relevant legislation/standards, the UHN Professional Practice Model and the mission and vision of nursing at the UHN to provide compassionate, collaborative and competent care, while meeting the legal and ethical requirements of the profession.

Duties

Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in the context of an interdisciplinary practice. Use current theoretical and research knowledge and the input of team members to plan and implement care for patients and families Participate in inter-professional team care planning forums, inter-professional rounds and family conferences Provide mentorship to RPN students and new staff on the unit Participate in professional, program and hospital-wide initiatives, including professional development of self Compliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients

Requirements

Candidates must hold a valid registration with the College of Nurses of Ontario and possess a Diploma in Practical Nursing. Recent related clinical experience is preferred, along with strong problem-solving skills and computer literacy.
