The Registered Nurse will conduct in-home, non-clinical assessments to review health history and functional needs. They are responsible for developing individualized care plans and serving as a clinical resource for the care team.

What you will be doing
Travel to clients’ homes to complete scheduled, non-clinical nursing assessments
Conduct verbal and observational assessments with clients and family members
Review medical history, diagnoses, medications, functional abilities, and daily routines
Identify risks, support needs, and appropriate non-medical services
Develop and update individualized care plans for in-home caregivers to follow
Serve as a clinical resource to the care team regarding the client’s plan of care
Maintain accurate and timely documentation of assessments and care plans
The focus of each visit is understanding the client’s health background, functional needs, and personal goals in order to build a realistic and supportive care plan.
