The Registered Nurse will provide comprehensive clinical care, including health assessments, chronic disease management, and travel health consultations. They will work within an interdisciplinary team to coordinate patient care, perform clinical interventions, and contribute to quality improvement initiatives.
What You'll Do You'll split time between one-on-one client consultations, care coordination with the clinical team, and quality improvement projects: Family Health Program:
Conduct comprehensive health assessments, preventative screenings, and wellness counseling Manage chronic and complex conditions; design and evaluate individualized care plans Perform clinical interventions: vaccinations, injections, cervical cancer screening, wound care, ECG, spirometry Triage urgent concerns and provide anticipatory guidance Coordinate community referrals and support care transitions
