This position will be part of the Intermediate Care Services within the Primary Health Care Networks. This service addresses a provincial health system service gap, where some clients with multiple complex health issues have difficulty in accessing an office based primary care provider. The goals for this service are: to provide the most at risk clients with home based access to higher level health care providers, including MDs and NPs; to mitigate the risk for worsening health sates and acute care episodes leading to Emergency department visits and hospitalizations for at risk clients; to reduce system pressures on acute care services, particularly Emergency departments and hospitals; to provide cost effective and high quality health care using patient centered and community based approach. Practitioners in this service are assigned to one of the Primary Health Care Networks and provide support to the interdisciplinary Primary Health Care team.

Requires a minimum of two years of recent related nursing experience and current registration with the CRNS. Must possess a valid Class 5 driver's license and BCLS certification.
