As a member of a multidisciplinary health care team the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting, including assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care. Travel may be a requirement of this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle; and Valid BC Driver's License.
Home Support, Resident Care Attendant, or Health Care Assistant certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience..
Food Safe Certificate..
Valid British Columbia driver's license..
Current registration in good standing with the British Columbia Care Aide and Community Health Worker Registry..
Ability to work independently with minimal supervision..
Effective verbal and written communication skills..
