The Diabetes Education Community Network (DECNET) is a partnership among South Riverdale Community Health Centre, East End Community Health Centre, Access Alliance Multicultural Health and Community Services, and Michael Garron Hospital. Working within the context of community health principles and practices, DECNET provides diabetes education support and services to adults with pre-diabetes and Type 2 diabetes in East Toronto. Particular emphasis is placed on increasing access for mental health, hard to reach, complex, and marginalized populations and members of our community. The Registered Practical Nurse (RPN) – Footcare – DECNET works as an integrated member of an inter-professional team within the DECNET team of SRCHC. The RPN performs assessments, develops client-focused treatment plans, provides health education/counselling, and other clinical activities. The RPN works within the scope of practice as outlined by the College of Nurses of Ontario (CNO) and must consult with an RN, NP, MD, or Chiropodist to manage complex client or medical presentations. This position will work collaboratively with other interdisciplinary team members, health care professionals / service providers and clients in the identification and resolution of health issues and integration of care, and strives for excellence in client centered care and education. Working onsite is required.
Complete intake and assessments as necessary e.g. health histories, social determinants of health, functional and family status, and physical assessments such as wound assessments, both in order to establish a plan of care within RPN scope of practice and to connect clients to additional services Provide footcare and treatment of foot-related health conditions in a supportive, therapeutic, and preventative means in order to attain/maintain optimal function of the client Perform foot/lower leg assessments, provide foot/lower leg treatment, health education and other clinical activities according to the College of Nurses of Ontario Professional Standards and within the scope of practice for RPN’s and Advanced Foot Care Nurse Certification Work independently, and under the guidance of Medical Directives, or in consultation with other SRCHC primary care providers or other relevant inter-professional team members as appropriate Educate and advise a wide range of clients, based on the client’s goals and practical ways in which they can improve their health by adopting healthier foot care/lower leg habits, facilitating self-management strategies, incorporating clients’ health goals, culture and income Work collaboratively with prescribing primary care providers (PCP’s), and other health care providers to provide optimal client care Review client footcare referrals from prescribing physicians, nurse practitioners and other professionals as well as the client’s eligibility for treatment and consult with the PCP/inter-professional staff Develop rapport with clients and maintains therapeutic relationships within professional boundaries Involve clients in all aspects of treatment and care Seek out opportunities to provide supportive counseling, education and referrals to services within and outside of SRCHC Support a welcoming, non-judgmental and safe space Support electronic scheduling Follow established documentation protocols and maintains clear, accurate and timely written documentation in accordance with procedures and policies of the College of Nurses of Ontario, Ministry of Health and Long-Term Care, Ontario Health and SRCHC Prepare/maintain statistical data, correspondence reports and other documentation, as required Support SRCHC’s research and evaluation activities; such as collecting, analyzing and reporting on data, participates in research as applicable Identify trends or issues related to the health status of priority groups and uses this information in service planning and advocacy work Work collaboratively with team members, participate in inter-disciplinary case conferences and team meetings, as required by SRCHC, and share information, and demonstrate and/or provide guidance on clinical/health related tasks Network with internal and community partners to ensure an optimal continuum of health for clients/patients Participate in the development, implementation, monitoring and evaluation of education, counseling and health promotion programs and services Collaborate with community partners to support population health from a footcare/lower leg health perspective Contribute to chart documentation, participate in chart reviews, case conferences and progress reports to referring/non-referring health care providers Maintain a hygienic working environment, conducts infection control tasks in accordance with related policies and procedures Assist with stocking, organization and inventory of supplies Preserve confidentiality and minimize risk Attend mandatory training as applicable Collaborate within own team and other teams across the organization to identify and provide health education to target groups Incorporate health promotion and harm reduction strategies and recognize the determinants of health, as well as the social and structural inequities that create the conditions of the social determinants of health All other duties as required
Diploma in Practical Nursing from a recognized College or School of Nursing Current unrestricted registration as a Registered Practical Nurse with the College of Nurses of Ontario or Registered Practical Nurses Association of Ontario (RPANO) Completion of Advanced Foot Care Nurse designation (or working towards) Current Basic Rescuer (C) certificate recognized by the Heart and Stroke Foundation of Ontario Currently a member with, or willing to join the Canadian Association of Footcare Nurses Medical reprocessing course(s) such as PHO, Canadian Standards Association course “Reprocessing Foot Care Devices”, IPAC Canada, CDMR or equivalent considered an asset Certificate of completion in one or more of the following complimentary specialty areas considered and asset: Diabetes education, Wound care, or Infection Prevention and Control (IPAC) 3 – 5 years nursing experience in a foot care/lower leg setting or in a community setting or combination of community, hospital or public health settings Strong knowledge of diabetes affecting adults with Type 1, Type 2, and prediabetes clients as well as other chronic conditions Experience working with clients 18 years and older with an emphasis on diabetes and other chronic diseases Knowledge of policies, legislation, programs and issues related to area of practice Thorough knowledge of community services and resources; strong advocacy skills Experience in program development, implementation, monitoring and evaluation Demonstrated ability to undertake footcare education Reprocessing experience an asset Proficiency in the use of computer technology and various software applications Demonstrated ability to de-escalate conflict Confidence in asserting and maintaining strong boundaries Strong oral and written communication skills Ability to actively and accurately listen Open to feedback Strong interpersonal skills Ability to be flexible Strong organizational skills Motivated, self-directed and a team player Respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others Demonstrated display of sensitivity and empathy to others Open, non-judgmental and willing to learn Respects and values the diversity of communities and individuals Fluency in a second language is an asset Police clearance documentation will be required for employment Proof of double COVID-19 vaccination status will be required for employment This posting is for an existing vacancy.
