The Registered Practical Nurse is responsible for providing nursing care, assessments, and goal-oriented treatments for patients in a rehabilitation setting. They also contribute to discharge planning and collaborate with an inter-professional team to achieve positive patient outcomes.
Responsible and accountable for decisions and actions of the provision and management of nursing care for patients; Provides nursing assessment, contributes to the care plan development, provides goal oriented treatment/interventions, patient and family education and support to address medical, cognitive, emotional and self-care needs; Contributes to the development of a realistic discharge plan Acts as a coach/mentor/consultant and uses sound critical thinking skills which are key to achieving patient outcomes and facilitating team functions; Participates in program planning, continuous quality improvement and evaluation. Key Qualifications, Skills and Abilities: Graduate of a recognized School of Nursing Current certification or eligibility for certification of competence from the College of Nurses of Ontario as a Registered Practical Nurse is required Current BCLS required Recent experience with inpatient medical population required Recent experience with inpatient rehab population and/or complex care needs population required Recent knowledge of the philosophy and concepts of geriatric nursing practice Demonstrates ability to provide patients and families with educational resources Demonstrates patient and family-centered care Demonstrates knowledge and ability to establish a therapeutic relationship Evidence of on-going professional development Must be reliable, flexible, creative, self-directed to work effectively and collaboratively in a high performance inter-professional team Satisfactory attendance and performance required Candidates are selected on the basis of their skill, ability, experience and qualifications.
