Functioning as part of the coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services, within as existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate and provides services in a non-judgmental and culturally appropriate manner, helping clients to build new skills and achieve their goals. This Program works within an Assertive Community Treatment Model and utilizes the Housing First Principles from a social justice orientation which includes harm reduction, pro-choice, social determinants of health, human rights, social development, population health, health equality and trauma informed, client centered practice.
Education Must have a Valid Drivers’ License and access to a reliable vehicle.
