The Case Worker is responsible for triaging referrals for the Ontario Structured Psychotherapy program and coordinating care with community partners. They also provide system navigation and support to patients, families, and care providers while maintaining safety standards.
The Case Worker in Ontario Structured Psychotherapy (OSP) reports to the Clinical Manager of OSP. The successful candidate will be part of a team responsible for the coordination of OSP including: triaging of all referrals received to OSP; communication with referral sources to ensure complete and appropriate information is received; presentation of complete referrals to intake clinician responsible for acceptance to the program.
Sound knowledge of community resources in mental health and addiction.
Demonstrated advanced knowledge of psychotropic medication.
Thorough knowledge and ability to administer mental status examinations, psychosocial and crisis assessments.
Demonstrated knowledge of the College Standards of Practice and commitment to maintaining reflective practice standards as described by the College.
Demonstrated knowledge of Mental Health, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required.
• 3-5 years psychiatric experience in a mental health facility or service.
