The Calgary Foothills PCN Coordinated Attachment program seeking locum physician

The Calgary Foothills PCN Coordinated Attachment [https://cfpcn.ca/coordinated-attachment/] team is seeking a family physician to join the program as locum coverage, with the possibility of an ongoing placement.

The program runs Monday through Friday between 8 a.m. and 4

30 p.m. Coverage could be between a 0.20-0.80 full-time equivalent (eight to 32 hours per week).

Coordinated Attachment is a medical home that panels patients with a variety of complexities, including chronic conditions, mental health concerns and social determinant of health needs. Referrals to this program come from hospitals, navigation coordinators, PCN programs and community partners.

The team includes physicians, a nurse practitioner, social worker, mental health clinician, registered nurse and licensed practical nurse.

This position is well suited to a practitioner who values a collaborative team environment wants to spend more time providing primary care services for patients with complex needs.
