About the role

Assists patients and their families with practical problems and personal concerns.

Consults with hospital staff and community agencies regarding psychosocial aspects of patients' situations.

Assists with complex discharge planning to return clients to their home communities.

Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Island Health (Vancouver Island Health Authority), patient & staff safety is a priority and a responsibility shared by everyone at VIHA; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position. Reporting to the Clinical Coordinator, performs professional social work of moderate complexity. Assists patients and their families with practical problems and personal concerns and consults with hospital staff and community agencies regarding the psychosocial aspects of the patients' situation.

Qualifications

  • Education, Training And Experience Bachelor of Social Work degree (B.S.W.) from an accredited school of Social Work, however a Master's degree (M.S.W.) is preferred. Current full registration with the BC College of Social Workers. Related social work experience, preferably in a medical setting, and demonstrated clinical competence. Skills And Abilities

Link to Full Job Description https://jd.viha.ca/JD767.pdf Additional information for applicants Currently this bachelor and/or masters degree of social work position is located on a Clinical Teaching and general medical unit at the Royal Jubilee Hospital and also provides short-term relief to other social workers on the same floor . Social Workers work in team-based care on a inter-professional health care team and work with a variety of clients and their families on complex discharge planning to return clients to their home communities. Social workers provide supportive counselling, assist clients with Advance Care Planning (financial management, health care consent and end of life decisions), housing, financial and other resource applications or referrals to support them back in their home community. Working knowledge of various pieces of legislation such as the Mental Health Act, Adult Guardianship Act and the Health Care Consent Act is required in this practice area. Previous experience working in acute care will be an asset. All social workers are Designated Responders under the Adult Guardianship Act and must investigate, assess and intervene in all matters related to Adult Abuse, neglect or Self-Neglect. Social workers work collaboratively with the Ministry of Children and Families in matters related to child protection. Social workers must be registered with the British Columbia College of Social workers to work in hospitals in B.C. Requirements

Bachelor of Social Work (B.S.W.) from an accredited school of Social Work (M.S.W. preferred).

Current full registration with the BC College of Social Workers.

Related social work experience, preferably in a medical setting.

Knowledge of community resources.
