Be skilled at coordinating and participating in case conferences in order to engage clients, keep families and other stakeholders informed, and create opportunities which allow clients to have meaningful input into their plans.

Be well-versed in negotiating case management plans; reviewing and determining suitable community services to assist clients with complex needs.

Adapt to a flexible schedule – occasional public presentations/meetings in the evening and on weekends.

Be willing to participate in an on-call/after-hours rotation (approximately one week per year).

Experience working with vulnerable adults such as persons with dementia, severe and persistent psychiatric disorders, developmental disabilities, and acquired brain injury.

Experience providing service delivery in accordance with legislation, associated regulations, and policies. (Adult Guardianship and Trusteeship Act, Personal Directives Act and Mental Health Act).

Experience working with court procedures.

Minimum recruitment standards outline the minimum education and experience required for appointment to a job classification.Notes Hours of work: 36.25 hours per week, Monday – Friday.
