Provide life skills training, coaching, advocacy, information, and support services to clients experiencing mental health, behavioural, and developmental problems in accordance with an established plan of care.
Deliver interventions focused on specific skills training and support needs, including relapse prevention, community adjustment, and employment skills.
Facilitate client empowerment and competency development within the four domains of psychosocial rehabilitation (personal life, education, leisure, and work).
Respect individual strengths, interests, capacity, and choice in all rehabilitation and recovery-oriented services.
Support skills transfer, generalization, and maintenance to maximize client growth and independence.
Assist clients in identifying and working through steps for success as rehabilitation and support needs are delineated by the client and team.
Monitor client goal attainment and proactively assist individuals in averting potential crises.
Report client status and progress to the Case Coordinator or multidisciplinary team, including noted reactions to medication.
Coordinate, plan, promote, arrange, facilitate, and evaluate specialized PSR programs such as Clubhouses, Supportive Housing, and Vocational Support Programs as required

Grade 12.
Diploma in a field relevant to the position (e.g., community social services, rehabilitation).
Two years recent related experience working with individuals experiencing mental health and substance use problems, or an equivalent combination of education, training, and experience.
Food Safe Certificate.
Current valid B.C.
Driver’s License (Class 5).
