About the role

The Licensed Practical Nurse provides direct patient care within an interprofessional team, including assessing, planning, and documenting client needs. They support clients in achieving optimal health while working under the guidance of a Registered Nurse and following established care plans.

minimum Nurse to Patient Ratios (mNPRs) Vancouver Coastal Health (VCH) is proud to be part of the provincial implementation of minimum Nurse to Patient Ratios (mNPRs), an initiative aimed at enhancing nursing practice and strengthening quality of care across the health system. mNPRs set the minimum number of nurses providing care to patients on a given unit.

In British Columbia, mNPRs are developed in partnership with the Ministry of Health, the BC Nurses’ Union (BCNU) and health organizations.

Salary Details The salary range for this position is CAD $32.84/Hr. - CAD $41.35/Hr.

Practices within the context of a client and family centred care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for Licensed Practical Nurses and the vision and values of the organization.

The LPN practices collaboratively as a member of an interprofessional team within Scope of Practice and Professional/Practice Standards of BCCNM for Licensed Practical Nurses and within VCH policies, guidelines and clinical practice documents.

The LPN provides care to clients and families based on predictable outcomes and the needs/outcomes can be managed by following an established plan of care or under the direction/supervision/guidance of a Registered Nurse (RN).

The LPN works closely with the RN in direct care activities such as assessing, planning, implementing, evaluating and documenting client care.

The LPN will support clients and families to achieve and maintain optimal health.

About Community Health Centers (CHC): CHCs offer clinic services, referral services, support groups, therapy and ambulatory nutrition care, and a full range of addiction and mental health services, to those who are living with complex psychosocial and clinical needs, who are vulnerable and underserved, and who require a higher intensity of services to achieve and maintain functional stability.

Each clinic is staffed with Integrated Care Teams (ICTs) to care for people who experience barriers accessing services, have no primary care providers and are challenged by complex health concerns (including psycho-social, physical, mental health, and addiction comorbidities).

Each ICT provides care on-site and via outreach, with nurses, social workers, counsellors, peer specialists, community liaison workers, familky physicians, nurse practisioners, and psychiatrists.

ICTs are supported on-site by more specialized care providers: wound care clinicians, palliative care nurses, OTs, dieticians, and many other disciplines.

Qualifications

Education & Experience Graduation from a recognized program for Practical Nurses. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years recent experience working as a Licensed Practical Nurse providing care in the community or acute (medical/surgical/palliative, simple wound care) care setting or an equivalent combination of training, education and experience. Valid B.C. Driver's License may be required as local area travel may require the use of a vehicle. Knowledge, Skills & Abilities Comprehensive knowledge of other health care disciplines and their role in client care. Demonstrated ability in dealing with a variety of situations and responsibilities requiring initiative, creativity and professional judgment. Knowledge and experience in providing culturally safe and competent care, specifically pertaining to Aboriginal people. Knowledge and practice within the standards dictated by the BCCNM for LPNs. Broad knowledge of working with clients and families across the life span. Broad knowledge of working with Medical, Surgical and Palliative clients. Broad knowledge of Community resources and polices. Demonstrated ability to apply decision-making and analytical skills. Demonstrated ability to work collaboratively as a member of an interprofessional team. Demonstrated ability to communicate effectively both verbally and in writing. Demonstrated ability to effectively deal with conflict situations. Demonstrated ability to plan, organize and prioritize work. Physical ability to perform the duties of the position. Knowledge and experience with basic computer programs (Word, Microsoft Outlook). Demonstrated skill in clinical techniques and the use of applicable equipment and supplies. Demonstrated skill in CPR techniques. Demonstrated computer skills including the ability to effectively use a computerized client care information system. Closing Statement The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.
