About the role

The Nurse Practitioner conducts clinical assessments, diagnoses, and treatments while providing health education and counseling to clients. They work collaboratively within an inter-professional team to provide optimal care and participate in community outreach to improve access to primary health services.

Position Title: Nurse Practitioner (2 Temporary Positions)

Organization: Windsor Essex Community Health Centre (weCHC), Leamington site (33 Princess St)

Classification: Temporary Full-Time (35 hours/week)

Salary Range

$66.32 – 72.63 / Hour

Position Type: Maternity Leave Backfill, up to 16-month contracts

Core responsibilities of the position

  • The primary focus of the Nurse Practitioner is to conduct assessments and work with the inter-professional team to provide care and treatment of health conditions. As a key member of the team, the Nurse Practitioner will:
  • Perform assessment, diagnosis and treatment, health education/counselling and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs) as well as the Medical Directives of the weCHC
  • Participate in active outreach in the region/community to assist client’s access to primary health care and serve as a connection to community physicians/inpatient services in hospitals
  • Work collaboratively with consulting physician and the health care team to provide optimal care
  • Provide clinic assessments addressing the physical and emotional needs of clients with multiple co-morbid conditions
  • Contribute to chart documentation as per college standards, participate in chart reviews and case conferences
  • Make referrals to support client needs and goals
  • Initiate the development, implementation, monitoring and evaluation of treatment, education, counselling and health promotion programs and services as appropriate
  • Implement appropriate strategies by working with the inter-professional team using a client centered approach
  • Actively participate in the teaching/learning process with clients, families, students and team members about best practice and evidence-based care
  • Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.
  • Skills, knowledge & experience required
  • Minimum 1-2 years experience as an Extended Class Nurse Practitioner working with vulnerable clients
  • Experience working with mental health and addictions is an asset.
  • Experience in a community health setting or combination of community health and hospital or public health settings
  • Strong attention to patient-centred care and establishing therapeutic relationships required
  • Demonstrated ability to work in an inter-professional team environment required
  • Strong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skills
  • Proficient in the use of computers and various software applications
  • Clean driver’s abstract, current, valid Class “G” Driver’s License and reliable transportation to support travel across Windsor-Essex County and the province, as required
  • Bilingual (English/Spanish) language skills a strong asset.
  • Education, training and certificate requirements
  • Undergraduate and/or Graduate degree in Nursing from a recognized University
  • Qualified to provide care to clients across their lifespan
  • Member in good standing in the Extended Class with the College of Nurses of Ontario
  • Member in good standing with the Nurse Practitioner’s Association of Ontario (NPAO) and/or Registered Nurses Association of Ontario (RNAO)
  • Training and/or certification related to mental health and addictions recommended considered an asset
  • Experience with phlebotomy
  • Completion of a Police Clearance and Vulnerable Sector Check prior to commencing employment.

Requirements

Candidates must hold an undergraduate or graduate degree in nursing and be a member in good standing with the College of Nurses of Ontario in the Extended Class. A minimum of 1-2 years of experience as an Extended Class Nurse Practitioner and a valid driver's license are required.
