About the role

The Social Worker will provide comprehensive clinical care and follow-up for pediatric patients and their families within a multidisciplinary healthcare team. Responsibilities include assessment, counseling, case management, and advocacy for patients dealing with acute and chronic medical illnesses.

As a member of the multidisciplinary healthcare team, the Social Worker will apply their clinical knowledge and skills to provide comprehensive care, and follow-up to children ages 0-18.

Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker. Master's Degree in Clinical Social Work from an accredited School of Social Work 3-5 years clinical experience (within the last 6 years) with children and families dealing with acute and chronic medical illness, death and dying, bereavement issues and chronic progressive illness Hospital experience is preferred Excellent knowledge of community support and placement services Knowledge of Healthcare Consent Act, Substitute Decisions Act and Child and Family Services Act. Proven ability to work as a member of interdisciplinary healthcare teams. Demonstrated skills in the performance of a therapeutic role including strong assessment, counselling, case management and advocacy skills Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem solve effectively Demonstrated commitment to ongoing learning Strong interpersonal, communication, teamwork, consultation and teaching skills

Requirements

Candidates must hold a Master's Degree in Clinical Social Work and a current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers. A minimum of 3-5 years of clinical experience with children and families in a medical setting is required.
