Clinical Responsibilities Complete comprehensive occupational therapy assessments and develop individualized treatment plans, goals, and discharge recommendations.
Conduct ongoing evaluation and modification of treatment plans based on client progress and changing needs.
Support clients transitioning from hospital to home through evidence-based interventions that promote safety, independence, and quality of life.
Collaborate with interdisciplinary team members to ensure coordinated and seamless care delivery.
