The Health Centre is currently recruiting for the following position:
Job Summary The Primary Care Nurse Practitioner role emphasizes comprehensive primary care, health promotion and disease prevention throughout the life span. As a member of our skilled, committed team, you will practice in a multifaceted role that includes clinical practice, education and consultation.
Perform assessment, diagnosis, treatment, health education/counselling and other clinical activities in accordance with the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs) and all relevant provincial legislation and regulations. Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities. Use a Harm Reduction approach with clients who may exhibit behaviours that place their health at risk. Triage client needs either over phone, or in person. Maintain accurate and up to date clinical notes in accordance with NP’s professional standards and organizational policy. Participate in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client populations as determined by the Health Centre. Work during both regular and extended hours (evenings/weekends). A minimum of one evening per week will be required. Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals. Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities. Work in a manner that preserves privacy and confidentiality. Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre. Work in a manner consistent with the Health Centre’s Vision, Mission and Values. Completion of a recognized Primary Care Nurse Practitioner program. Master’s degree in nursing; or equivalent recognized education and experience. Current certification with the College of Nurses of Ontario-Extended Class and a member in good standing. 2 years minimum experience as a Primary Care NP working with all ages in a community health setting. Valid Ontario Drivers License. Knowledge of the health delivery system, community and social services and the relevant legislation related to Nurse Practitioners in Ontario. Proven experience working in a collaborative team environment; and Demonstrated leadership in promoting a shared responsibility for professional practice and development. Knowledge of cultural diversity as well as newcomer and refugee health in Canada is desirable. Ability to communicate in another language is an asset. Experience working effectively with culturally and economically diverse Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre. 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.
37.5 Hours per week for 18 month contract Monday/Tuesday/Thursday 9am-5pm Wednesday 9am – 5pm or 12 pm – 8pm Fridays 8am – 4pm
Requires a Master's degree in nursing, completion of a Primary Care NP program, and current CNO-Extended Class certification. A minimum of 2 years experience as a Primary Care NP in a community health setting is required.
