The Emergency Department Admission Avoidance Program seeks to identify, strategize and implement discharge planning as soon as a patient arrives at St Joseph’s Healthcare Hamilton. The Admission Avoidance team allocates in-house, community and provincially funded resources to get patients home sooner and straight from the Emergency Department

The Admission Avoidance team includes Registered Nurses, Physiotherapists, Occupational Therapists, Social Workers, Community Care, Coordinators, Physicians, Families and Caregivers
The Registered Nurse (RN) is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the SJHH mission, vision and values, policies and procedures; and applicable specialty standards. The RN provides patient/client-centered and evidence informed care to promote health and assess the need for the provision of care and the treatment of health conditions. The RN collaborates with the inter-professional team and assumes a leadership role in the delivering a full range of care and treatment to patient/clients including those with complex needs, high acuity and less predictable outcomes.
Requires current registration with the College of Nurses of Ontario and BCLS/HCP certification. Candidates must possess strong clinical assessment skills and expertise in managing complex patient needs, including geriatric and mental health care.
