Company Description Make an Impact Through Community-Based Primary Care Nova Scotia Health is seeking a Family Physician to join the Nova Scotia Brotherhood Program on a 0.2 FTE basis.
The Nova Scotia Brotherhood was created to support Black and African Nova Scotian men in overcoming barriers to healthcare through culturally responsive, community-based care.
The program provides primary care services, chronic disease management, health promotion, wellness programming, and navigation to community resources within trusted community settings.
This is an opportunity to be part of an innovative interdisciplinary team focused on improving access to care and advancing health equity for Black and African Nova Scotian communities.
The Opportunity Working within the Family Medicine Alternative Payment Plan (FM APP) model, the successful candidate will provide comprehensive primary care services through community-based clinics located throughout the Halifax Regional Municipality.
Clinic locations rotate according to patient need and appointment schedules and currently include: North Dartmouth and North Halifax East Preston, North Preston, Cherry Brook, and Lake Loon Hammonds Plains Dartmouth (230 Wyse Road) The physician will work collaboratively with an interdisciplinary team to provide culturally safe, patient-centred care in locations that are familiar and accessible to members of the communities served.
Provide comprehensive family medicine services to patients enrolled in the Nova Scotia Brotherhood Program. Diagnose and manage acute and chronic medical conditions. Deliver preventative care, health promotion, and chronic disease management. Collaborate with an interdisciplinary team that may include nursing, wellness navigation, mental health professionals, and community partners. Support culturally responsive and trauma-informed approaches to care. Contribute to improving health outcomes and healthcare access for Black and African Nova Scotian men. All applicants must be eligible for licensure with the College of Physicians and Surgeons of Nova Scotia (CPSNS) All applicants must be eligible for certification with the College of Family Physicians of Canada (CFPC) Membership with the Canadian Medical Protective Association (CMPA) Demonstrated commitment to health equity, cultural competence, and community engagement. Experience working with underserved or equity-deserving populations is considered an asset.
