Assess, coordinate, and deliver primary therapeutic, case management, and clinical occupational therapy services for assigned clients in community settings.
Provide consultative services to clients in acute care or tertiary settings.
Deliver services in accordance with care plans developed by the multidisciplinary clinical team, the individual, and/or the family.
Work as part of a therapeutic team including the Clinical Director and Coordinator.
Manage a case management caseload embedded on an Assertive Community Treatment (ACT) team.
Provide consultation for other ACT teams as required.
Provide 80% outreach services to individuals with severe and persistent mental illness and significant cognitive impairment.
Work closely with vocational rehabilitation specialists and support group work requirements.
Prepare and maintain charts, records, and reports related to work.

Under the administrative direction of the Coordinator and as a part of the therapeutic team which includes the Clinical Director, the Case Manager is responsible for assessing, coordinating and delivering primary therapeutic, case management and clinical occupational therapy services for assigned clients in community settings and providing consultative services to clients in acute care or tertiary settings. Such services are provided in accordance with care plans developed by the multidisciplinary clinical team, the individual and/or the family.
Graduation from a recognized university program in Occupational Therapy.
Current licensure in a practicing category with the College of Health and Care Professionals of BC as an Occupational Therapist.
Eligible for membership in the Canadian Association of Occupational Therapists (CAOT).
Knowledge of current therapy standards, modalities, and scope of practice.
Two (2) years' recent related experience, including experience working in a mental health and addictions environment (or equivalent combination of training and experience).
Valid BC Driver's License.
Ability to integrate and apply COTBC Standards for Occupational Therapy Practice in community mental health and addictions practice.
Clinical understanding of affective disorders, schizophrenia, serious psychiatric disorders, and their treatment using a comprehensive bio psychosocial model of care.
Clinical understanding of addictions diagnosis and treatment when addictions co-occur with serious psychiatric disorders.
Ability to work in an outreach case management modality.
Ability to work effectively as part of a multi-disciplinary team.
Ability to deal effectively with a wide range of community and facility contacts (patients/clients, families, staff, physicians, psychiatrists, community and public agencies).
Ability to organize work effectively, establish and carry out priorities to meet critical deadlines.
Ability to communicate effectively verbally and in writing.
Demonstrated ability to prepare/maintain a variety of charts, records/reports.
Familiarity and understanding of relevant legislation pertaining to mental health and addictions.
Travel between multiple sites is a regular/on-going requirement; transportation arrangements must meet VIHA operational requirements.
