About the role

Maintain good standing with the College of Physicians within the Province of Ontario.

Provide move-in and annual physical examinations and ongoing medical orders for residents.

Document resident assessments and maintain complete health records at move-in and each subsequent visit.

Collaborate with the Village multi-disciplinary team to provide guidance for medical care plans for assigned residents.

Provide medical direction to neighbourhood teams.

Deliver standard on-site hours and reasonable after-hours and on-call services to the village and arrange coverage when not available.

Participate in the Medical Advisory Committee and Quality Improvement initiatives.

Collaborate with clinical and medical leadership to develop, implement, monitor, and evaluate Village Medical Services.

Assist in village-wide issues such as infection control management and outbreak management.

Support MLTC compliance reviews.

Participate in care planning with families, residents, and a multi-disciplinary team.

Demonstrate knowledge of relevant policies, procedures, legislation, and regulations applicable to the long-term care sector, including the Fixing Long-Term Care Act, 2021 and Health & Safety.

Support and embrace a resident-centered social culture and deliver high-quality clinical care.

Collaborate with the village team and the medical director on quality resident care and support mentorship and education of clinical team members

Requirements

Good standing with the College of Physicians within the Province of Ontario.

Current knowledge of geriatrics, gerontology, and chronic disease management.
