Time Commitment

An Intensive Case Manager is expected to work 37.5 hours per week, primarily Monday to Friday with the possibility of working occasional evening or weekend shifts dependent on program needs.

Objective

Following Housing First principles, and as part of Mission Services’ Housing UP team, the Intensive Case Manager will provide ongoing support to individuals to secure permanent accommodations and maintain their housing. The Intensive Case Manager will work as a part of the Housing UP team and will engage regularly with service users in the community to provide the necessary support to achieve program and participant goals.

Responsibilities

and Duties:

  • Assists in the engagement of prospective program participants who are identified through the Coordinated Access System
  • Assesses participant's needs for services to support their transition to housing and optimize their capacity to sustain housing using an acuity tool and other resources
  • Document and input information into service user files, manually and in computer databases, to ensure accurate service statistics and service user records
  • Develop, implement and monitor case plans and sustainability plans
  • Maintain regular, effective communication with social housing providers, administrators and private landlords, to ensure an integrated approach that will provide the best support possible for participants to maintain housing
  • Liaise with income support programs (i.e. OW, ODSP, CPP etc.) to assist in establishing an income source for participants
  • Assist each program participant with securing appropriate housing based on individual needs
  • Assist participants with their move into independent accommodations and provide neighborhood orientation
  • Provide enhanced support with maintaining safe housing and rapid re-housing where needed
  • Improve links with social, health, income and employment supports, as they relate to participant’s housing plans
  • Adhere to a strict policy of confidentiality
  • Work safely, identify any health and safety concerns and report to management, and follow Mission Services workplace health and safety policies and procedures at all times
  • Other duties as required to ensure the continuation of services

Qualifications

  • 1. Education
  • Minimum post-secondary degree or diploma in social work, social service work, or a related field
  • 2. Experience
  • Minimum one to three (1-3) years of related experience; three to five (3 to 5) years of experience is preferred
  • Experience working with individuals with mental health and addictions issues
  • 3. Knowledge
  • Understanding of the impacts of addiction, mental health, homelessness and poverty
  • Thorough knowledge of various income support programs (i.e. OW, ODSP, CPP etc.)
  • Familiar with local community

Requirements
