Your role

Contribute, review and implement person-centered plans designed to support and assist adults, in achieving their personal goals

Collaborate with other staff to facilitate meaningful, high quality community connections, personal development and recreation opportunities.

Encourage and support individual independence, choice, and self-expression in the areas of self care, communication, recreation, socialization and participation.

Act as a role model while encouraging and assisting the individual to participate in the daily activities and responsibilities of their personal life and their home

Support residents to participate and engage in the community through leisure activities, and fostering social networks that align with the individual's interests and person centered plan

Establish and maintain rapport with residents and maintain positive relationships with key stakeholders

Keep accurate records and complete necessary documentation requirements, including but not limited to Person Centered Plan updates, daily logs and incident reports.

What you will need to be successful

Post secondary education and/or experience in Human Services and/or disability support with preference given to individuals with a background in Person Centered concepts.

A positive attitude and belief that persons with disabilities have the right and responsibility to define and pursue their life choices and pursue active participation and involvement in the community.

Be willing to transport Supported individuals in your vehicle to appointments, activities and events.

Preference to individuals that have certification in PART, Food Safety, TLR, Medication Administration.

A satisfactory Criminal Record Check including Vulnerable Sector Search

Must have a valid Class 5 Saskatchewan Driver's License

Must have a reliable vehicle that you are able to use for work each day, including $2,000,000 liability insurance.

Please note: Supports are provided 24 hours a day, 7 days per week. Shift work is required to include both Saturdays and Sundays.

Requirements
