Plan, implement, evaluate, and coordinate patient/client access and care transition activities between Interior Health facilities and health care services across the continuum of care for assigned patients/clients with complex care planning and health service needs.
Work collegially and collaboratively with multidisciplinary/multi-agency teams.
Act as a resource to patients/clients/families and health service teams/professionals in assessing patient/client needs and eligibility for appropriate services/resources.
Operate within competency guidelines and full scope of practice per BCCNM Standards of Practice and organizational/unit-specific policies, procedures, standards, and protocols

Graduation from a recognized program for Practical Nurses.
Three (3) years recent related clinical experience including a combination of Community and Acute Care nursing experience with the assigned complex patient/client population (or equivalent combination of education, training, and experience).
Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) required.
Current valid B.C.
Drivers License required
