About the role

Provide culturally informed routine nursing care and home support services to community members. Collaborate with a multidisciplinary team to monitor patient health, administer medications, and manage patient travel services.

Position Summary

Seabird Island is committed to providing routine care to our clients. We are seeking a Licensed Practical Nurse to join our team, working collaboratively to improve the quality of life for clients and community members within the communities we serve.

Under the direction of the LPN & Home Care Team Lead the Licensed Practical Nurse provides culturally informed routine care, observing patients’ health, assisting doctors and registered nurses, and communicating with patients and their families. The Licensed Practical Nurse will also engage in community relations activities to support clients and the community.

What You'll Do Advise community members of available health services and programs through NIHB and medical services.

Perform nursing functions using established procedures, policies, guidelines, and standards.

Contribute to the development of the patient/significant other plan of care and discharge plans based on the identified patient problems/needs.

Document pertinent observations and interventions on appropriate records.

Conduct the initial home visit and refer clients to HCC and/or appropriate services.

Administer medication accurately, observing patient response, as evidenced by documentation in the medical record.

Primary Care includes performing dressing changes and monitoring vital signs etc.

Report client needs/problems and observations regarding their condition to the registered nurse and physician.

Administer the patient travel services according to the patient travel policies and procedures.

Maintain and distribute clinical supplies.

If needed, assist in coordinating accommodation for community members traveling for health reasons.

Perform foot care procedures for clients, as required.

Identify gaps in services and inform the Manager and the Department Team.

Provide Home Support services including meal preparation, personal care, palliative care, transportation, and transfer of functions as requested by the HCC/CHN nurse.

Provide follow-up visits to monitor the health of clients including pre-natal, hospital discharge and other illnesses as requested by HCC/CHN nurse.

Maintain professional and accurate client service records.

Prepare all necessary documentation for community members as needed.

Be familiar and updated with Health Canada policies and procedures governing the delivery of community health services.

Other Duties Additional responsibilities as needed to support the community and uphold our shared values.

Working / Special Conditions Initial and ongoing employment is subject to submission of a clear Criminal Record Review Program Check to ensure the safety and trust of the communities we serve.

This check will be conducted every five years.

Initial and ongoing commitment to the values and guidelines of Sq'éwqel (Seabird Island Band), including our policies, procedures, standards of practice, and laws, is essential.

This commitment supports our collective work, the community, and the responsibilities of this role.

Must possess a valid class 5 driver's license and have access to safe and reliable transportation, insured for community-based use, to effectively transport clients.

What You Bring Completion of Licensed Practical Nursing Program.

Current registration with the BCCNM.

Valid First aid Certification with CPR.

Excellent communication and relationship-building skills.

Ability to work with community members, health professionals, and co-workers as a member of a multidisciplinary team.

Preferred

Indigenous candidates are strongly encouraged to apply. Lived experience as a member of a First Nations community. Knowledge and understanding of Seabird Island’s culture, traditions, teachings, community dynamics. Ability to respond quickly and professionally to community or colleague needs and inquiries. Excellent writing, editing, and verbal communication skills. Direct experience in a First Nations health services position or community public health environment.

Cultural Competency

Understanding of Indigenous cultures, traditions, and contemporary issues. Experience working within Indigenous communities and applying culturally safe practices. Understanding of intergenerational trauma and its impacts on Indigenous peoples and communities.
