We are hiring a permanent Case Manager on our Assertive Community Treatment Team (ACTT) – South Simcoe

Closing Date: July 31, 2026

Position

1-Year Contract, full-time, working 35 hours per week. Evening hours and weekends may be required.

Salary

$56,444-68,429 per annum, *Candidate experience will be considered when determining the starting salary.

Location

The Case Manager will be working primarily in our Alliston Office located at 35 Victoria St E, Alliston ON L9R 1T3 and out in the community and will be required to travel within York Region and South Simcoe to visit clients and travel to clients homes.

Job Profile

The Case Manager works with clients 16 years of age and older who are experiencing significant difficulties in day-to-day and role functioning as a result of serious and persistent mental illness and/or addictions. The Case Manager collaborates with the client to identify strengths and needs and develop long term goals, short term objectives to meet these goals, and concrete action steps to achieve the objectives. As part of this process individuals are assisted to acquire and use skills, and to connect with formal and informal supports, peer support, professional services, and resources to promote their recovery, improve their quality of life, and reduce the likelihood of relapse. Other duties as required.

The Case Manager collaborates with clients to provide individualized and flexible services to engage, support, and integrate clients into the community, and facilitate their use of resources and supports to maximize their independence and potential. The Case Manager adheres to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety. The Case Manager participates in individual and systemic advocacy initiatives to improve resource access and availability.

In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.

Requirements

  • A degree or diploma in the health or social services. Preference is given to a degree in a related discipline.
  • Full registration and in good standing with the OCSWSSW or a regulatory body under the Regulated Health Professions Act
  • One or more years of experience in a community-based organization working with adults with serious mental illness in the community.
  • Second language (Cantonese, Mandarin, Farsi or Tamil, Italian, Korean, Punjabi, Russian, Urdu) reflecting the local community is an asset.
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPPA requirements.
  • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with clients.
  • Extensive knowledge of supports and services in York Region and South Simcoe, including formal and informal resources.
  • Knowledge of systemic issues such as poverty, unemployment, stigma, medications and the isolation felt by individuals with serious mental health issues and their families.
  • A valid Canadian Full G driver’s license, a minimum $1,000,000 third party liability insurance.
  • A satisfactory Vulnerable Sector Screening (Police Check)

Responsibilities

  • Is familiar with, and adheres to Intensive Case Management Services Standards of the Ontario Ministry of Health and Long Term Care (October, 2012).
  • Maintains a client caseload in accordance with the above standards, recognizing the need for extensive travel in some rural areas and intensiveness of contact with specific clients.
  • Is familiar with signs and symptoms of trauma in clients and is able to respond appropriately in making referrals and providing short-term assistance to clients to help them to manage these symptoms.
  • Performs comprehensive assessments.
  • Initiates referrals to other CMHA programs or external agencies as required.
  • Anticipates, understands, and responds to the needs of internal and external clients within organizational parameters in order to meet or exceed their expectations.
  • Establishes a collaborative partnership with clients that involves non-judgmental listening and client-directed goal setting that fosters independence, self-determination, competence and hope, by employing empowerment/Recovery principles and practices.
  • Uses effective listening skills to address the clients’ issues and needs.
  • Informs clients of confidentiality requirements and limitations of confidentiality.
  • Assists clients in developing individual empowerment plans that include discussion of their strengths and needs, setting of long and short-term goals, establishing methods to address the goals, and creating target gates.

Working Conditions

Desk work up to 7 hours a day in standard shared office environment, meetings clients in community and their homes, extensive travel in York Region and South Simcoe, evening and weekends may be required.
