Help patients regain independence at home. As an OT in our Integrated Care Program, you’ll support safe transitions from hospital to home through functional assessments, personalized interventions, and collaboration with care teams — making a real impact where it counts.
Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
