About the role

The Community Health Worker conducts outreach and engagement in priority neighbourhoods to link residents with primary care, mental health, and social supports. They act as a bridge between the community and available services while collecting data to inform service improvements.

Hours of Work

Up to 6 hours per week.

Hourly Rate

$21.50 per hour

The Community Health Worker supports community outreach, engagement, and health navigation activities within priority neighbourhoods across Windsor and Essex County.

Through relationship-building, community presence, and trusted conversations, the Community Health Worker acts as a trusted community connector, helping to link individuals to primary care, mental health, and community-based supports.

Working in community settings such as neighbourhood events, community agencies, public spaces, housing sites, faith-based organizations, and other outreach locations, the Community Health Worker acts as a bridge between residents and available community supports.

The role promotes awareness of services, identifies barriers to access, supports referrals and resource navigation, and helps bring community voice and lived experience into service planning and improvement efforts.

Community Health Workers play an important role in strengthening trust, improving access to care, increasing awareness of available services, and helping build healthier, more connected communities.

RESPONSIBILITIES & DUTIES: Conduct community outreach and engagement activities within designated neighbourhoods and community settings, including door-to-door outreach, public spaces, housing sites, faith-based organizations, community events, and virtual platforms.

Provide information, education and support to community members, assisting them in accessing relevant resources and services, including primary and preventative care, mental health support, and recovery information.

Conduct community outreach and engagement activities to identify resident and community needs, strengths, priorities, and barriers to accessing services, informing targeted interventions and support strategies.

Assist community members in navigating health, social, and community support systems.

Facilitate referrals and warm connections to appropriate services and supports, including primary care, mental health, housing, food security, employment, and other community resources across Windsor- Essex county.

Conduct regular face-to-face or virtual interactions or group-based information sessions with residents as part of community outreach, engagement, and service navigation activities.

Attend meetings, training sessions, professional development opportunities, and team activities, as required.

Work collaboratively with local organizations, community groups, and other stakeholders to deliver wraparound services and achieve shared goals.

Represent the CMHA in a professional, respectful, and culturally responsive manner consistent with its mission, vision and values.

Communicate emerging community needs, barriers to access, and opportunities for service improvement to program staff and communicated partners when suitable.

Facilities or support outreach campaigns, community events, presentations, information sessions, and other engagement activities and social events, as required Collect, maintain, and submit accurate activity, outreach, referral, and engagement data in accordance with program requirements.

Qualifications

  • High school diploma or experience deemed relevant by the Agency At least 1-2 years of experience in community outreach, social services, or a related field Demonstrated experience working with diverse populations and understanding community health issues.
  • Familiarity with local resources and services available to residents is an asset.
  • Given the nature of this position, it is a requirement that the incumbent resides in one of the following postal codes: N8X, N8Y, N9A, N9B, N9C, N9Y or N8H Ability to communicate in languages commonly spoken within Windsor-Essex communities (e.g., Arabic, Spanish, French, Punjabi, Urdu, Tagalog, Mandarin, etc) is considered a strong asset.
