GCHC prioritizes people who face barriers to health and accessing services, and people who identify as: Homeless or at risk of homelessness Living on low income Newcomers to Canada with language barriers Having moderate to severe mental health and/or addiction issues Having moderate to severe disabilities Indigenous First Nations, Inuit and Métis Black, mixed race and racialized populations Experiencing intersectional oppression 2SLGBTQIA+ Vulnerable children and their families experiencing conflict, isolation or attachment struggles Working as part of an interdisciplinary team to provide clinical health services for clients and members of our priority populations, the Nurse Practitioner (NP) provides specialized health care with a strong emphasis on health promotion and disease prevention within the scope of practice and standards of care outlined in the Expanded Nursing Services for Patients Act and Regulations and the Practice Expectations Specific to Registered Nurses in the Extended Class (NPs).

The NP diagnoses and manages common health problems and diseases both independently and in collaboration with the client and the physician.

The Nurse Practitioner works as part of an interdisciplinary team to address issues in the community that affect health.

This role could include work at any GCHC site and could also include community work at partner locations.

Main Responsibilities Provide ongoing, comprehensive health care to clients within priority population group, including addressing the health care needs of clients.

This may include comprehensive assessments, diagnosis, counselling, screening, referral, education, treatment and follow-up, supportive client counseling, and crisis intervention.

Work within a determinants of health framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors (social, physical, psychological etc.) which may affect the health of clients Respond compassionately to the needs of clients, with an understanding of how poverty, language barriers, mental illness, cognitive abilities and homelessness contributes to health outcomes.

Address stigma and reduce barriers to health care access wherever possible Assure standards of patient care are maintained in accordance with accepted current best practice guidelines & GCHC policy.

Maintains client confidentiality and acts in an ethical and professional manner in accordance with the codes of conduct prescribed by the College of Nurses and in accordance with the Centre's policies.

Key Qualifications and Skills Registration in the Extended Class with the College of Nurses of Ontario Member of the Nurse Practitioners Association of Ontario Experience working with communities and individuals who face barriers to accessing health care due illness and other factors known to be the determinants of health Previous experience in community health/social services and/or a values-based organization is an asset Able to independently manage complex clients with comorbidities and vulnerabilities, consulting as appropriate Ability to speak multiple languages is an asset Proficient in the use of computers and various other software applications including Word, Outlook, PSS and electronic health records Understand and are committed to the Mission, Vision and Values of GCHC.

Note

Nurse Practitioner may be required to share on‑call duties with other providers in the future and may be required to work offsite at community locations

Requirements

Must be registered in the Extended Class with the College of Nurses of Ontario and a member of the Nurse Practitioners Association of Ontario. Experience working with marginalized communities and proficiency in electronic health records are required or preferred.
