Monday - Friday 2PM-10PM

Brief overview

Under the direction of the Mobile Outreach Project Manager, the Mobile Outreach Support Worker is responsible for supporting the Health and Mental Health & Addictions department, conducting assessments, and identifying mental health needs, advocating, mentoring, and connecting with patients through support groups and social services.

Key responsibilities

  • Conducting initial assessments to identify patients' mental health needs and develop individualized care plans.
  • Coordinating and monitoring patient care with other healthcare professionals, including psychiatrists, therapists, and primary care providers.
  • Providing education to patients and their families on mental health issues, treatment options, and community resources.
  • Advocating for patients to ensure their needs are met and their rights are protected.
  • Monitoring and recording patient progress and adjusting care plans as necessary.
  • Connecting patients with community resources, such as support groups and social services.
  • Maintaining accurate and up-to-date patient records.
  • Other duties as assigned.

Qualifications

  • Certificate/Diploma in the field of Mental Health and Addictions, Social Work, Psychology, or related field.
  • Minimum 1-3 years of experience in the Mental Health and Addictions field.
  • Experience providing case management services to individuals with mental health issues.
  • Valid First Aid/CPR.
  • Familiarity with electronic medical records (EMR) systems.
  • Valid driver's license and a satisfactory drivers abstract is required.

Knowledge and skills

  • Understanding of the factors that contribute to mental health and wellness and addiction or risk of becoming addicted, understanding the unique challenges of working and supporting people living with mental health and/or addiction, and understanding our citizens needs by listening to their voice, is the basis of this program, delivering a distinct based mental health and addiction outreach with the focus on our Métis citizens while providing the same supports inclusively to community members experiencing mental health and addiction challenges.
  • Sound knowledge in the determinants of health.
  • Ability to work in a low barrier housing environment with sensitivity to the population of shelter and supportive housing environments.
  • Ability to work in a harm reduction environment which includes an observed consumption area, as well as familiarization with the principles of harm reduction.
  • Physical abilities to carry out the duties of the position.
  • Ability to operate related equipment.
  • Ability to establish and maintain rapport with clients, staff and management and collateral service providers.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
  • Strong problem-solving and critical thinking skills.
  • Ability to work independently with minimal supervision.
  • Knowledge of Métis culture.

Additional requirements

  • Must undergo a Criminal Record and Vulnerable Sector Check.
  • Valid Saskatchewan driver’s license and reliable transportation.
