Provide client-specific care coordination within the Yukon Home Care Program.
Provide direct nursing care including education and treatment related to medical conditions (e.g., wound care, ostomy care).
Collaborate with a multidisciplinary/interdisciplinary team (physiotherapists, occupational therapists, licensed practical nurses, social workers, home support workers).
Develop and support individualized care plans.
Submit funding requests and perform other nursing duties.
Provide professional care that promotes clients’ physical, emotional, and spiritual well-being.
Provide leadership, coaching, and training to team members.
Operate across multiple levels of care within a transdisciplinary environment
Registered Nurse registration with a Canadian registered nursing regulatory authority.
Recent and relevant clinical nursing experience within the Registered Nurse scope of practice (assessment, care planning, implementation, evaluation).
Home Care and/or Community Care nursing experience.
Ability to provide professional care promoting physical, emotional and spiritual well-being.
Ability to provide leadership, coaching and training to team members.
Effective oral and written communication skills.
Excellent organizational skills.
Experience working in a community care setting.
Excellent critical thinking, relationship building and conflict management skills.

Strong understanding of working with various cultural backgrounds.
TB screening; Vulnerable Sector RCMP security clearance; CPR Level C with AED (within last 30 months); Transportation of Dangerous Goods (within last 30 months); Valid class 5 Driver’s License without driving restrictions; Current registration with the Yukon Registered Nurses Association
