About the role

Provide client-specific care coordination within the Yukon Home Care Program.

Provide direct nursing care including education and treatment related to medical conditions (e.g., wound care, ostomy care).

Collaborate with a multidisciplinary/interdisciplinary team (physiotherapists, occupational therapists, licensed practical nurses, social workers, home support workers).

Develop and support individualized care plans.

Submit funding requests and perform other nursing duties.

Provide professional care that promotes clients’ physical, emotional, and spiritual well-being.

Job Summary and Requirements This is a until –filled competition. Your role The Yukon Home Care Program provides client-focused health services in the home, improving quality of life and supporting Yukoners to live independently. Home Care offers a range of services tailored to meet individual needs, including support for acute care, chronic and palliative conditions. These services are determined through standardized assessments to assist in creating individualized plans of care; and are supported by an interdisciplinary team, consisting of Physiotherapists, Occupational Therapists, Licensed Practical Nurses, Social Workers and Home Support Workers. As an integral team member, the Registered Nurse service delivery includes client specific care coordination, liaising with the medical community direct nursing care, such as; education, treatment related to medical conditions (i.e. wound care, ostomy care etc.), collaboration with a multidisciplinary team, development of care plans, funding requests, and other nursing duties.

Requirements

Registered Nurse registration with a Canadian registered nursing regulatory authority.

Recent and relevant clinical nursing experience within the Registered Nurse scope of practice (assessment, care planning, implementation, evaluation).

Home Care and/or Community Care nursing experience.

Ability to provide professional care promoting physical, emotional and spiritual well-being.

Ability to provide leadership, coaching and training to team members.

Effective oral and written communication skills.
