The Nurse provides patient-centered primary health care services in collaboration with a multidisciplinary team. Responsibilities include performing nursing interventions, administering medications, and maintaining accurate patient records in the EMR.

The Role The Nurse works in collaboration with physicians, nurse practitioners, and other independent health professionals to provide patient-centred, quality primary health care services.
Within the context of a patient-centred care model, and working within a multidisciplinary, inter-organizational team, the Nurse will: Provide nursing services, within defined scope of practice, to patients based on patients' assessments and treatment plans; Perform nursing interventions such as taking blood pressure and other vital signs, applying aseptic techniques such as sterile dressings, ensuring infection control, monitoring nutritional intake and conducting specimen collection; Administer medication and observe and document therapeutic effects; Monitor patients' progress, evaluate effectiveness of nursing interventions and consult with appropriate members of the health care team; Call patients for follow up and communication and review Maintain effective communication with all team members; Participate in multidisciplinary team meetings regarding planning, implementation and revision of patient care programs; Optimize patient safety; Maintain clear and concise patient care records in EMR Assisting other team members when needed Must Haves Graduate with Degree in Nursing Current registration with the College of Nurses of Ontario.
Current membership with the RNAO Previous related experience in a primary care setting is preferred.
Ability to work collaboratively within an interdisciplinary team, as well as to work independently.
Excellent verbal, written and interpersonal skills.
ACLS, BCLS for Healthcare Providers with AED Training. Proficiency in the use of computers and electronic medical charting. Available to work evenings. Valid driver’s license, access to a reliable vehicle, willingness to provide home visits. Satisfactory Criminal Record Check
