About the role

Provide direct medical and practical support to individuals with addiction and mental health issues within a supportive housing program. Responsibilities include conducting health assessments, managing substance use disorders, and collaborating with a multi-disciplinary team.

Stonehenge Therapeutic Community provides a range of residential and community-based treatment services to individuals struggling with addiction and concurrent mental health issues. About the Role

The Supportive Addiction & Mental Health Housing program supports individuals living in the Guelph-Wellington community who are dealing with significant addiction and mental health issues (as well as other complexities), to gain housing stability and enhance their quality of life by providing housing, practical support, counselling, advocacy, landlord mediation, and eviction prevention.

The successful candidate has a passion for Addictions Medicine and embraces an approach focusing on health promotion and risk reduction. Through multi-agency collaborations and working alongside SAMH staff, the RPN will play a vital role as a member of our multi-disciplinary team, offering a range of practical and medical supports to clients. RPN staff provide direct patient care, working within the full RPN scope of practice. This role is a mobile role, requiring regular travel within the Guelph-Wellington area.

Location and Hours of Work Information

  • Monday to
  • Friday, days (typical hours are 8:30am - 4:30pm). Participation in the extended hours rotation is also required, where RPN staff work one 11am-7pm day/wk in place of day hours.

Two (2) Positions Available - 1 based out of Guelph; 1 based out of Mount Forest, working within the Rural Guelph-Wellington area

Qualifications

Practical Nursing Diploma from a CNO-recognized program Registered Practical Nurse in good standing with the College of Nurses of Ontario (CNO) required; Member of RPNO an asset Evidence of Professional practice liability insurance (minimum $2 million) Minimum two years of progressive experience in addictions or mental health; Experience in inpatient or community setting preferred One year of recent experience in withdrawal management preferred Experience in concurrent disorders or trauma informed practice an asset Knowledge of suicide prevention and crisis intervention strategies Lived experience with addictions and/or mental health an asset Valid Ontario Driver’s License, with reliable (own) transportation Satisfactory Motor Vehicle Record Proof of valid Ontario driver’s license (G2 or G) Reliable vehicle and willing to transport program participants Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) acknowledgement of scope of vehicle use by Insurance provider Vulnerable Sector Screening check required pre-employment; PWSG Clearance may be required at the discretion of the Agency Satisfactory reference check required pre-employment Current First Aid and CPR certification required pre-employment May be required to meet occupational health and policy requirements of partner Agency locations including full COVID-19 vaccination, annual influenza vaccination and full immunization series

Role Overview

Direct Client Support Assess, plan, implement, and evaluate bio-psycho-social-spiritual care needs in accordance with best practices; document electronically and/or manually. Conduct health assessments to evaluate emotional, cognitive, and behavioural states, including crisis levels, aggression, self-harm, suicide risk, risk to others Assess and manage clients with substance use disorders and co-existing conditions; complete screening tools for mental health, screening for sexually transmitted and blood borne infections. Assess and manage substance use and substance use disorders including completion of validated scales and reviewing diagnostic criteria Complete physical health assessments and focused physical health interventions e.g. wound care Provide nursing interventions and treatments as ordered Respond to medical emergencies and initiate appropriate crisis interventions Collect specimens (i.e. blood, urine, blood glucose) as required In live-in settings, assist with management of client medications including liaising with community pharmacies, medication record reconciliation administration and documentation Identify learning needs and provide health related education Collaboration & Documentation Complete all documentation in accordance with STC policies and the College of Nurses of Ontario (CNO) standards Participate in interdisciplinary case conferences and contribute to collaborative care planning Participation in regular team meetings Provide input into program development, policy updates, and quality improvement initiatives Perform additional related duties as assigned to support program operations and client care Provide support to Supportive Housing RPN team (holiday coverage)

Requires a Practical Nursing Diploma and registration in good standing with the College of Nurses of Ontario. Candidates must have at least two years of experience in addictions or mental health and a valid Ontario Driver's License with a reliable vehicle.
