About the role

Provide supportive services during awake overnight shifts for people residing in the home in accordance with policies, procedures, and legislative requirements.

Act as the support person using a Personal Outcome Measures lens.

Support individuals to develop and realize individualized goals through the Individual Support Planning process.

Complete required documentation in a timely and effective manner (Documentation Due reports) and report concerns to the Supports & Services Supervisors.

Actively participate in supporting individuals to achieve personal outcomes.

Provide guidance and direct support for activities of daily living, personal care, communication, emotional/physical/mental health, safety, finances, and relationship development to foster independence.

Provide personal care when needed and perform visual checks throughout the night.

Maintain vigilance over physical and mental well-being; promptly report changes to the On-site coordinator and update individual records.

Monitor behaviour, psychoactive medication usage, possible blood sugar levels, and other tracking; provide feedback and record information to the team and in the Electronic Medication Administration System.

Facilitate family involvement where appropriate and requested.

Work with the On-site coordinator/team to provide crisis intervention and assist individuals in accessing community supports as appropriate.

Maintain a clean, safe, and secure workplace (laundry, sweeping/mopping, nightly cleaning checklist).

Complete nightly fire and safety checklist each shift.

Prepare and serve meals and beverages in a polite and courteous manner

We are presently accepting resumes for a Full-Time Direct Care Staff- Awake Overnight to provide supportive services during awake overnight shift to meet the needs of the people in accordance with the policies and procedures of Empower Simcoe and legislative requirements. Compensation package:

Work up to 72 hours biweekly;

Minimum starting rate of pay is $25.66 per hour; - 11 paid statutory holidays;

Employer paid confidential Employee Assistance Program for you and your family members; Minimum requirements for this position include:

Proven successful completion of grade 12 high school diploma and a PSW Certificate. Other post-secondary academic credentials maybe considered for equivalency as determined by the employer.

Previous recent work experience in a community residential setting providing support for individuals with disabilities is preferred and a definite asset.

Knowledge and familiarity with computer operations and software applications.

Current valid driver's licence and own reliable transportation to be used in the course of employment. Overview of duties (including, but not limited to):

Using a Personal Outcome Measures lens, act as the support person for people residing in the home

Supporting people to develop and realize individualized goals through the Individual Support

Planning process in accordance with the desire of the person and in conjunction with the Supports and Services Supervisor and the Senior Support Worker

Completing all required documentation in a timely and effective manner, as outlined in Documentation Due reports and as requested by the Supports and Services Supervisor

Actively participating and supporting the person to work toward achieving their personal outcomes

Providing personal care when needed and visual checks throughout the night to all people supported in the home

Maintaining vigilance over the physical and mental well-being of individuals, promptly reporting any changes in condition to the On-site coordinator, while accurately and diligently updating individual records as necessary

Monitoring person's behaviour, psychoactive medication usage, possible blood sugar levels and other tracking as needed and provide feedback and recording to the team and on the Electronic Medication Administration System. Requirements

Grade 12 high school diploma and PSW Certificate (other post-secondary credentials may be considered for equivalency as determined by the employer).

Previous recent work experience in a community residential setting supporting individuals with disabilities is preferred and an asset.

Knowledge and familiarity with computer operations and software applications.

Current valid driver's licence and own reliable transportation for use in the course of employment.

Current valid certificates in First Aid, CPR, Non-Violent Crisis Intervention (N.V.C.I), and medication dispensing OR successful completion of a recognized pharmacology course.

Understanding of The Council on Quality and Leadership and Personal Outcome Measures is an asset
