Reporting to the Manager, Pharmacy Clinical Programs, or designate, the Clinical Pharmacist, Community Health Services works collaboratively with the Seniors, End of Life and Home & Community Care teams, is responsible for practicing client centered care by identifying and resolving potential drug therapy problems in the community setting; provides medication counseling and drug information to clients and health care providers; conducts clinic sessions and/or home visits as required; and provides academic detailing to optimize prescribing practices.
Licensed by, or eligible for licensing by, the College of Pharmacists of British Columbia.
Completion of an accredited pharmacy residency.
Three (3) years recent pharmacy practice experience, including community-based practice or an equivalent combination of education, training, and experience.
Ability to identify and resolve drug therapy problems for elderly and palliative clients.
Ability to apply clinical knowledge and evidence-based decision-making.
Ability to establish and maintain effective working relationships with healthcare practitioners and care providers, as well as clients, family members, volunteers, and the public.
Ability to communicate effectively verbally and in writing.
Ability to be self-directed, organized, prioritize workload, and complete tasks accurately and dependably.
Ability to teach.
Ability to be flexible and adapt pharmacy practice to a variety of community settings.
Ability to physically carry out the duties of the position.
Valid BC drivers license.
CRC required for professional registration
