Responsibilities
- Conduct comprehensive health assessments, including medical histories and physical examinations in collaboration with cardiologists and Internal Medicine specialists.
- Provide patient education and counseling regarding disease prevention, treatment options, and healthy lifestyle choices.
- Monitor patient progress and adjust treatment plans as required.
- Collaborate with physicians, specialists, allied health professionals, and community resources to ensure continuity of care.
- Maintain accurate and timely clinical documentation within the electronic medical record (EMR) system.
- Participate in quality improvement initiatives and support best practices in patient care.
- Adhere to all regulatory, professional, and organizational standard
- Perform consultations with patients as assigned.
- Perform stress test supervision as assigned.
- Perform all medical tasks delegated by the attending physician.
- Contribute to ensuring there is a safe environment for patients, staff and visitors.
- Maintain safe and clean working environment by complying with procedures, rules, and regulations.
- Call for assistance from health care support personnel, if required.
- Document patient care services by charting in patient records.
- Take all patient vitals and history.
- Maintain patient information electronically.
- Document and record medicine and prescriptions.
- Perform any and all other tasks as assigned by the supervisor.
Qualifications, Experience & Requirements
- 0-2 years of related experience.
- Nurse Practitioner certification is required.
- Member in good standing with the College of Nurses of Ontario.
- Demonstrated knowledge and understanding of general medicine.
- Demonstrated ability to collaborate and communicate effectively with health professionals and patients.
- Must possess the ability to function effectively in a clinical setting.
- Familiarity with electronic medical records (EMR).
- Knowledge of and demonstrated ability in corporate core competencies including customer service, communication, teamwork, initiative/self-management, accountability, dependability, flexibility and adaptability.
- Computer literacy utilizing MS Office software applications and ability to research on the Internet.
- Ability to work within a diverse community and to deal empathetically with patients, family members, etc., to contribute to their well-being.
- Ability to work effectively independently or as a part of a multidisciplinary healthcare team.
- Demonstrated commitment to continuing education and training.
- Demonstrate effective decision-making and problem solving skills.
