Woolwich Community Health Centre (WCHC),is a multi-site health centre and requires a Nurse Practitioner (22.50 hours per week, 0.6 Permanent FTE, Temporary Contract for 6 months at our Wellesley site). All WCHC Nurse Practitioners work to their full scope of practice within an inter-disciplinary team to provide comprehensive primary health care to a registered client caseload. In addition to providing comprehensive primary care, within this unique role, the Nurse Practitioner will also engage in community-facing programing aimed at reducing primary care unattachment rates across rural Waterloo Region and surrounding areas. Woolwich Community Health Centre is responsible for program and service delivery to clients who are residents of Woolwich, Wellesley, and Wilmot Townships.
