Job description

  • Salary The salary range for this position is CAD $32.84/Hr. - CAD $41.35/Hr.
  • Job Accuracy Posting Flag The hours of work, including start and stop times, days off, work area and work site may be subject to change consistent with operations requirements and applicable provisions of the Collective Agreement and statutes. **Testing may be required. ***A criminal record check may be required Summary Under the guidance of the Resident Care Manager and in close collaboration with a diverse healthcare team, the Licensed Practical Nurse (LPN) delivers top-notch nursing care to long-term care residents in addition to providing emotional comfort and support to residents and their families.

  • Holy Family is recognized for excellence in rehabiltation care provided by an interdisciplinary team of physicians, nursing staff, occupational therapists, physiotherapists, and Resident Care Attendants.
  • Holy Family also has 4 beds dedicated to patients undergoing peritoneal dialysis.
  • Minimum Nurse-to-Patient Ratio Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit.

Your Day to Day Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team Administer medications per protocol, monitor effects, and maintain communication with the team Utilize evidence-based resources to stay updated on nursing practices Respond to incidents such as codes, injuries, spills, and alarms, following procedures Assist with staff orientation, demonstrating procedures and providing support Address patient/resident and family learning needs, provide educational materials, and make referrals Attend care planning meetings, provide input, and advocate for patients/residents Collaborate with the team to develop and evaluate individualized care plans Assist with discharge planning based on patient status and external supports Operate and clean equipment, report malfunctions, and handle hazardous materials.
