The Clinical Integration Pharmacist provides direct patient care and medication management expertise within an integrated primary care team. Key duties include conducting medication assessments, managing chronic diseases and addictions, and coordinating care transitions for vulnerable populations.
Candidates must hold a BSc. Pharm or PharmD, be registered with the Alberta College of Pharmacists, and possess Additional Prescribing Authority and Authorization to Inject. A minimum of three to five years of clinical pharmacy experience is required, with a preference for expertise in addictions medicine.

The Clinical Integration Pharmacist (CIP) of the Edmonton North Primary Care Network (PCN) is an integrated primary care role embedded within the physician clinic. Serving patients experiencing health and social inequities in Edmonton, the pharmacist works with interdisciplinary teams to support complex physical, behavioural health, medication-related, and social needs. The pharmacist coordinates clinic medication management systems; serves as a dedicated clinical resource in addictions medicine, infectious diseases, and chronic disease management; supports transition of care, including hospital discharge follow-up; and liaises with community pharmacies and other healthcare partners to improve medication access, continuity, adherence, and therapeutic outcomes.
The Clinical Integration Pharmacist provides direct patient care, medication management expertise, and leadership in the development of safe and effective medication systems within the clinic. The role supports patients through medication assessment, prescribing and deprescribing activities, chronic disease management, addiction medicine, transitions of care, medication access, patient education, and interdisciplinary consultation.
The Pharmacist also contributes to quality improvement initiatives, program development, learner preceptorship, and the ongoing evolution of integrated primary care pharmacy services.
