The Discharge Planner coordinates timely and safe patient transitions from the hospital to the community through collaborative interdisciplinary planning. They conduct in-depth assessments and address psychosocial barriers to ensure patients receive the right care in the right setting.
A bachelor's degree in Social Work, Nursing, or a Health Discipline is required, with a master's degree preferred and current professional registration. Candidates must have 2-5 years of acute care experience and essential expertise in discharge planning and psychosocial assessments.

Under the leadership of the Manager, Discharge Planning, the Discharge Planner will work within a fast paced environment as a collaborative team member of an interdisciplinary team. The primary goal of Discharge Planning services is to support and enable patients and families to make timely transitions through the hospital system. The Discharge Planner does all of this in an effort with the goal of providing an exceptional patient and family experience. The Discharge Planner understands the Flow and Capacity challenges of a hospital system and works to achieve that the patients are in the right place, at the right time, receiving the right care.
