The Social Worker provides clinical and consultative services to clients and families, focusing on psychosocial assessments and counselling. They play a key role in the HOPE team, overseeing intake and intervention to help clients maintain housing and prevent eviction.
Requires a Bachelor's degree in Social Work and current registration with the British Columbia College of Social Workers. Candidates need at least two years of experience working with marginalized clients and a valid BC Driver's License.
Within the context of a patient/client and family-centred framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, the Social Worker is responsible for providing clinical and consultative social work services to clients and families. Duties include conducting psychosocial assessments, providing counselling services, maintaining records, attending interdisciplinary clinical/case conferences, identifying applicable community resources for clients and facilitating care throughout the continuum.This work is framed within VCH’s commitments to our pillars of Indigenous Cultural Safety, Anti-Racism, Equity, Diversity and Inclusion, and Planetary Health.QUALIFICATIONS:Education & Experience Current full registration with the British Columbia College of Social Workers.Bachelor’s degree in Social Work from an approved school of Social Work. Two (2) years’ recent related experience working with marginalized clients with multiple health issues (i.e., HIV and mental health and substance use issues) in community based or Indigenous-led or First Nations organizations, or an equivalent combination of education, training and experience. Valid BC Driver’s License and access to a personal vehicle for business-related purposes, as required.Knowledge & AbilitiesBasic knowledge of systems thinking as it relates to social work process.Working knowledge of counselling skills.Understanding of local (i.e., Downtown Eastside) and other regional initiatives that may affect patient care, resource access or practice guidelines in this local clinic setting.General knowledge of Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.Demonstrated ability to promote Indigenous Cultural Safety. Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations.Working knowledge of other health disciplines and their role in healthcare.Working knowledge of legislation and government policies and procedures that relate to the client.Demonstrated ability to conduct psychosocial assessments.Demonstrated ability to apply systems theory to practice in Social Work assessment and intervention.Demonstrated ability to draw out and honour information that will contribute to understanding the clients’ situation.Demonstrated ability to communicate effectively both verbally and in writing with clients, families, team members and community agency personnel.Demonstrated ability to adjust to new or unexpected events.Demonstrated ability to deal effectively with conflict situations.Demonstrated ability to work collaboratively as a member of an interdisciplinary team.Ability to operate related equipment including applicable software applications.Physical ability to perform the duties of the position.The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Details The Social Worker is part of HOPE (Housing Outreach Preventing Eviction), a team that supports clients in Vancouver to maintain their housing. HOPE focuses on offering a short-term intervention based on intensive deep-cleaning and/or household management supports that address immediate risks of eviction, life safety issues, or barriers to discharge from hospital. The HOPE Social Worker plays a key role in overseeing the intake, assessment, intervention, and discharge planning for all HOPE clients.
